Juliet delivered him. He is still alive because all woman and children were evacuated from the island on the sub prior to the bomb going off. Which explains why Ben is back too. It seems though that because the island blew up Jacob never left to change their lives so things are happening a little differently in this time-line. (Jack's son, Sawyer wearing glasses, Shannon NOT being on the plane, Desmond being on the plane, etc). And if Sayid and Claire are sick from the temple water... then that means Ben is too (in both time-lines) because that's how the Others saved young Ben when Sayid shot him...
